This PR implements an optional SQLite back-end for grounding terms. The SQLite database can be generated from the default tsv.gz grounding entries and then used to instantiate a Grounder instance (instructions are added to the README). This optional mode results in close to instant startup time, and - assuming no context-dependent disambiguation is used - minimal memory usage.
Resolves #95.
